We create a dedicated plan of care for each of our patients who come to us for short-term rehabilitation.
To help ease the process and alleviate some of the stress we offer the warmest and most caring social service team you’ll ever meet. Our social workers will provide answers to all your pressing questions and will be responsible for seeing that your loved one receives the care and resources they so richly deserve.
Our social service team will create a holistic and comprehensive plan of care that meets the needs of your loved one. To begin, they will initiate a formal assessment to determine their physical, emotional, and social needs, which will allow them to design a robust care plan. Your family will be kept in the loop every step of the way and our social workers will perform regular checks and assessments to ensure that the plan is effective and carried out properly. When your loved one is ready to be discharged back to home, our social service team will meet with you again to ensure their smooth and seamless transition back to home with all the appropriate support they are entitled to as they reacclimate to living independently.
