Introducing Outdoor Visitation

Guidelines for Visitation

Although we are still restricted to indoor visitation in the facility, in compliance with the Centers of Medicare and Medicaid (CMS) Guidance, we have considered all risk factors, and have developed a plan to initiate limited visitation, outdoors, with strict guidelines for our residents to be able to interact and have in-person visits with family and loved ones.

Limited visitation will be allowed for any residents who are NOT currently on isolation for COVID or quarantine for possible COVID contact.

These visitation guidelines require strategizing on our part to create a fair and manageable system. We will ask for your patience in scheduling, as staff members will be assigned to oversee, coordinate, and attend these visits. The guidelines we have set up are in place according to CMS and Department of Health and are required to be followed. We ask for your understanding. We will be restricting in-person individual visits to 30 minutes on Mondays through Friday only, provided you have an appointment.

How to schedule an appointment

All visits require an appointment, which can be scheduled through the Receptionist (845-278-3636) Monday through Friday between the hours of 8:00 am – 4:00 pm. The appointment calendar is now open. Please call to arrange your visit!

Location for outside visits

Apple Unit – 10:30am – 11:00am and 3:30pm – 4:00pm (Outside Apple Unit on the patio with the gazebo)

Birch Unit – 10:30am – 11:00 am and 3:30pm – 4:00pm (Adult Day Care underneath the signs “Everyday Heroes Work Here”)

Cedar Unit – 11:30am – 12:00pm and 4:30 pm – 5:00pm (Outside of Great Room near the gazebo)

Dogwood – 11:30am – 12:00pm and 4:30 pm – 5:00pm (Outside Dogwood Day Room on their patio)

Visitation Process

Wearing a facemask is mandatory upon entering the building.

Facemasks will be made available. Face mask/covering must cover both the nose and mouth. They must be worn throughout the check in process and DURING the outside visit with your loved one

a) All visitors to enter through the main lobby doors.

b) Use hand sanitizer provided

c) Receptionist will take your temperature and record it. Anyone with a temperature of 99.6 or higher will not be permitted to visit.

d) Check in on the kiosk. The following documentation must be completed and available upon the Department of Health’s request for purposes of inspection and potential contact tracing. Documentation must include the following for each visitor.

    • First and last name of the visitor;
    • Physical (street) address of the visitor;
    • Daytime and evening telephone number;
    • Date and time of visit;
    • Email address if available.

f) The Receptionist will then direct you to the designated location for the visit.

Important tips for visiting

Arrive 10-15 minutes before your scheduled appointment. This will allow time to check in at the Reception Desk to complete the necessary questionnaire and drive to the visiting location. The half hour visiting time will not be extended; therefore, it is very important you arrive on time.

Social Distance is to be maintained throughout the outside visit

You may bring your loved one a package or edible treat.  Please be advised that all food related items will need to be consumed after the visit as masks are to be worn throughout the visit.

You will not be allowed to visit without a scheduled appointment.

You do not need to bring a chair.

Only 2 visitors per resident during scheduled visit. Visitors under the age of 18 must be accompanied by an adult.

New admissions are considered under quarantine for the first 14 days of their admission and are not allowed visitors during this time.

In an effort to accommodate all friends and loved ones of our residents, you will only be able to sign up for one visit per week.

Please understand that the staff member providing supervision at the visits will likely not have clinical information about your loved one. If you have questions or concerns about your loved ones health and psychosocial well being, please speak directly to the unit staff.

 

We are very excited to welcome you back into the building.  The residents have missed you….so have the staff!
